<p><strong>JOB TITLE: </strong>Licensed Practical Nurse</p>
<p><strong>EMPLOYMENT TYPE: </strong>Part-time, 20 hours/wk</p>
<p><strong>SCHEDULED HOURS: </strong>Every Saturday 5:30am until 12pm, Two Rotating days during the week 5:30am until 12pm</p>

<p><span style="text-decoration: underline;"><strong>ABOUT THE PROGRAM:</strong></span><strong> </strong><strong></strong><strong>Pathways Opiate Treatment Program</strong></p>
<p>Pathways is an accredited Opiate Treatment Program specializing in Medication-Assisted Treatment (MAT), exclusively using methadone. Our comprehensive approach includes:</p>

<li style="vertical-align: middle;"><span>Psycho-educational groups</span></li>
<li style="vertical-align: middle;"><span>Individualized treatment planning</span></li>
<li style="vertical-align: middle;"><span>Recovery coaching and harm reduction services</span></li>
<li style="vertical-align: middle;"><span>Ongoing assessments and referrals</span></li>
</ul>

<p><span style="text-decoration: underline;"><strong>ABOUT THE POSITION:</strong> </span><strong>Licensed Practical Nurse</strong></p>
<ul></ul>
<p><strong>Duties & Responsibilities</strong></p>
<ul>
<li>Administer daily MAT (including methadone and buprenorphine) in accordance with physician/APRN orders and regulatory standards.</li>
<li>Prepare and dispense individual doses, ensuring accuracy and adherence to chain-of-custody procedures.</li>
<li>Safely transport and secure controlled medications between CHR OTP clinics, mobile clinics, and correctional facilities in compliance with policy and regulatory requirements.</li>
<li>Monitor patient response to treatment, including conducting assessments and observing for side effects or adverse reactions.</li>
<li>Conduct and monitor toxicology screens.</li>
<li>Maintain accurate and timely documentation in the electronic health record (EHR), consistent with program and agency standards.</li>
<li>Manage medication inventory, including ordering, restocking, and reconciliation, in compliance with CHR and regulatory requirements (DPH, DMHAS, DEA, Joint Commission).</li>
<li>Communicate effectively with prescribers, clinical team members, and DOC staff regarding patient care, progress, and safety issues.</li>
<li>Participate in team meetings, supervision, trainings, and other required activities.</li>
<li>Ensure compliance with all DOC, CHR, and regulatory safety/security protocols.</li>
<li>Perform other related duties as assigned.</li>
</ul>
<p><span style="text-decoration: underline;"><strong>QUALIFICATIONS:</strong></span></p>
<p><strong>Education:</strong> Licensed Practical Nurse (LPN) diploma/certification required or Nursing education sufficient to receive licensure in CT.<br /><br /><strong>Licensure/Certification:</strong> Current LPN license in the State of Connecticut required; valid driver’s license required.<br /><br /><strong>Experience:</strong> Prior experience in MAT, substance use treatment, or correctional healthcare preferred.</p>

<p>$22.72/hour minimum, and up commensurate upon experience</p>
<p>*Actual rates are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked) </p>
